Why I collect your information
I collect personal information and health information so that I can assess you, provide safe and appropriate acupuncture treatment, keep accurate clinical records, communicate with you about appointments, take payments and meet my professional, legal and insurance obligations. Health information is treated as special category data and is given additional protection.
What information I collect
This may include your name, contact details, date of birth, appointment history, medical history, symptoms, medicines and supplements, treatment information and clinical notes. I may also keep information relating to payments, invoices and communications about your care. Most information is provided directly by you.
How I use your information
Your information is used only where necessary for the purposes described above. I do not sell your personal information or use your health information for advertising. Appointment reminders and routine communications may be sent by email, text or telephone, according to the contact details you provide.
Who I may share information with
I will keep your information confidential. Where necessary and lawful, relevant information may be shared with another healthcare professional involved in your care, or with professional advisers such as my insurer or accountant. I may also use trusted service providers for booking, payment, email, secure storage or IT support. Information may be disclosed where I am legally required to do so or where necessary to protect someone's vital interests.
How I protect and retain your information
I take reasonable technical and organisational measures to protect your information from loss, misuse, unauthorised access or disclosure. Paper records are stored securely and electronic records are protected by appropriate access controls. I retain clinical records for as long as necessary to meet my professional, legal and insurance obligations, and securely delete or destroy information when it is no longer required.
Your rights
You have rights over your personal information, including the right to request a copy of the information I hold about you and, where appropriate, to ask for inaccurate information to be corrected, processing to be restricted or information to be deleted. These rights are subject to legal and professional requirements, so they do not apply in every circumstance.
